Need a vaccination Summary for my pet?

 The quickest answer for this is the LCAH app, which is downloadable at the Apple Store or Google Market Place. If you need the vaccine summary immediately and have not used the LCAH app then I would suggest calling the clinic. 

 From the app's Home Screen My Pets icon is the middle icon.

Click on My Pets icon, which will take you to My Pets. Select the pet you want the vaccine summary for. There are 3 tabs at the top above the picture of your pet. Select the About tab, below your pet's name select Request Records. The Pet's Name will already be filled out, fill in where you want the summary to be sent and select Request Record. 

 Yes, below your pet's name, there is a "Send Record to Email" box below your pet's name. 

Once you enter the email address select Vaccination Summary and select Request Record.  So easy your pet could do it.
